Finalize Form 1094-C

Form 1094-C Finalization

Each time you generate Form 1095-C, the corresponding Form 1094-C is generated

Before finalizing Form 1094-C, review the 1094-C page and revise Parts I, II, III, and IV and update data, when needed. Once updates are complete, finalize and certify Form 1094-C for each ALE. Then, download, distribute, print, or file your forms with the IRS.

Update Part II - Applicable Large Employer Member Information - Certifications of Eligibility

Review the certifications of eligibility and designate (if applicable) if the ALE is eligible to use the Qualifying Offer Method to report the information on Form 1095-C or the 98% Offer Method.

Update Part III - Minimum Essential Offer Indicator

The Minimum Essential Offer Indicator value is determined based on the percentage of full time employees and types of offer of coverage codes reported by the auto-population feature. When offer of coverage codes were imported or manually entered, update values based on the reported codes, when needed.

A value of Yes is reported in cases where the Section 4980H Transition Relief option is not selected in Part II of Form 1094-C and the following conditions apply:

  • Employer has offered coverage to at least 95% of full time employees.
  • 95% of the employees listed under the Full Time Employee Count have an offer of coverage code of 1A, 1C, 1E, 1F, 1K, 1M, 1N, 1P, 1Q, or 1R reported, or have a 2E safe harbor code reported.
    • Part time employees who have had their offer of coverage codes imported or manually entered are not included in the MEC indicator calculation or the full-time count, unless they have been overridden to ACA Full Time status.
    • Employees who have had their Form 1095-C suppressed are not included in the MEC indicator calculation or the full-time count.

  1. Navigate to Form 1094-C page.
  2. Select Edit and update the value.
  3. Select Save.

  2. Audit the data to ensure it is accurate.
  3. Select Edit and update the number count value, when needed.
  4. Select Save.

Review the list of all employees and update, when needed.

  1. From the Total Employee Count column, select the Download Data link.
  2. Audit the data to ensure it is accurate.
  3. Select Edit and update the number count value, when needed.
  4. Select Save.

Finalize Form 1094-C

Each time you generate Forms 1095-C, the corresponding Form 1094-C is generated. You must finalize and certify Form 1094-C for each ALE before you can download, distribute, print, or file your forms with the IRS.

The Finalize Form 1094-C page displays these form statuses:

  • Ready to Finalize

The Ready to Finalize status appears when all Forms 1095-C, related to an ALE Member, do not contain generation errors and the entire year is populated in the Part III Monthly Info section of Form 1094-C.

  • Finalized
  1. From Reporting, select Generate and Finalize. The Generate, Finalize, and Distribute Forms page appears.
  2. In the Finalize 1094-C section, select Continue. The Finalize 1094-C page appears. Only ALEs that are in Ready to Finalize status or are in the process of being finalized display on this page.
    Note

To finalize and sign Form 1094-C, web access rights must be enabled for ACA administrators under role administration (Reporting > PPACA > Generate and Finalize > Finalize 1094-C).

  3. From the Finalize 1094-C page, select the applicable ALE name in a Ready To Finalize status.

    Form 1094-C appears.

  4. Select Edit.
  5. Review the 1094-C page and revise Parts I, II, III, and IV, as needed.
  6. In the Finalize and E-Sign section, check the I Agree (acknowledgment) box.
  7. Enter your name and job title.
  8. Select Finalize and E-Sign. The In Progress message appears.

In some cases Form 1094-C does not finalize due to additional errors found during the final validation process. Should this occur, return to the Generate and Finalize page. Verify the error file, make corrections, and re-generate the ALE.

After finalizing and e-signing Form 1094-C, administrators can then distribute, self-print, or approve Forms 1095-C for Print Services. Administrators can also self-file or approve to have UKG e-file Forms 1094-C and 1095-C.

    Note

The ALE is automatically created for the next filing year after Affordable Care Act (ACA) administrators complete finalization of the current year's ALE (Reporting > PPACA > Generate and Finalize > Continue > Finalize 1094-C).
