San Francisco Health Care Security Ordinance (HCSO)

The HCSO requires that “covered employers” make specified “health care expenditures” on behalf of “covered employees.” Covered employees include any employee working in San Francisco who was employed for 90 days and who worked 10 or more hours per week (eight hours in 2009). Employers are required by law to contribute to health care for their San Francisco employees.

Note: Choose those component companies that are part of the organization that is doing business in San Francisco. The Date criteria used is the Reporting Year as of date.

The following table describes the rows in the report.

 A

Average number of employees per week.

§       The report determines if employees are active (EmpHStat) during a given week and if they received a paycheck where the period dates cover that week.

§       The report accounts for partial weeks and defines a week as Sunday – Saturday.

§       Report accounts for employees who are paid on different pay frequencies.

§       This figure is for all component companies selected, this count is not only San Francisco employees.

 B1

Total number of covered employees.

§       Employee must be “employed” 90 calendar days. Employment does not have to be continuous, consecutive, nor completed during the same calendar year.

§       Employees who “work” 10 hours (8 hours in 2009) per week during quarter are covered.

§       Only those earnings marked as include in “hours worked” for HCSO are included.

§       Earnings such as vacation should not be marked as hours worked.

 B2

Employees subject to exemption categories.

§       Count of those employees marked as not covered under HCSO.

§       If employees are marked at any time during the quarter as not covered, then they are considered as not covered for that quarter.

 C

Total number of hours paid to all covered employees.

§       Only hours for earnings marked as include in “hours paid” for HCSO are counted as hours paid for covered employees.

§       Monthly and quarterly limits on hours are respected (172 and 516).

§       The pay date determines the quarter that the hours are counted towards.

 D

Total amount of health care expenditures made for all covered employees. For covered employees, this includes the employer portion of deductions paid for the following deduction types: Dental, Medical, Prescription Drug, Vision, Health Reimburse Arrange, and Health Savings Account.

 E

Total number of employees for whom health care expenditures were made. Employees are counted once per quarter.

 


Voluntary Self-Identification of Disability

For compliance with the Department of Labor's Office of Federal Contract Compliance Programs (OFCCP) for the Vietnam Era Veterans Readjustment Assistance Act (VEVRAA) and Section 503 of the Rehabilitation Act, employers who work as federal contractor establishments can use this standard report for audit reporting and viewing or printing all Voluntary Self-Identification of Disability forms submitted by employees through UltiPro. Employers are required by law to collect, store, and maintain data to show proof of compliance with OFCCP requirements in the event of an audit.
